Prevenza specializes in provisioning of physician-led, transitional care services delivered into the home during the transition of care from a hospital, managed facility, or group setting. We partner with home care service agencies to provide excellent coordination of care during the service period following discharge to ensure each of our patients receives the support and aid needed to maximize recovery and improve quality of life.

Timely review of orders, signatures, plan-of-care updates and supporting physician documentation which reduces clinical and administrative delays. Shared clinical notes and dedicated patient care coordinators reduces rework, unblocks care and accelerates billing.

Direct HH provider access and dedicated care coordinators reduce time nurses spend chasing orders, clarifying medications and coordinating escalations. This frees nursing capacity for skilled care and reduces overtime and administrative expense.

* Preventable Hospitalization and HHVBP Performance -

Physician-led transition management, medication reconciliation, condition-specific care plans and early intervention help stabilize high-risk patients, including ratings and scoring. This supports quality performance, continuity of home health care and future Medicare payment gains.
