When One Door Closes, Another Opens: Pivoting With Purpose in Florida's Changing Home Care Landscape

By Denise Bellville, RN, BS, Executive Director

August often represents a season of transition. Families prepare for a new school year, businesses evaluate the first half of the year, and leaders begin planning for what comes next. This year, however, many home care entrepreneurs are facing a different kind of transition — one driven by federal policy.

The nationwide moratorium on new Medicare-certified home health and hospice enrollments, which includes Florida, has undoubtedly altered the landscape for organizations seeking to enter or expand within the traditional Medicare market. For many, this announcement has been disappointing. Business plans have been delayed, acquisitions have become more attractive than startups, and uncertainty has replaced excitement.

Although the enrollment moratorium is expected to be temporary, it presents real short-term challenges for entrepreneurs and organizations planning to enter the Medicare-certified market.

But if there is one thing Florida's home care providers have consistently demonstrated, it is resilience.

A Time to Rethink, Not Retreat

The demand for care has never been greater.

Every day, thousands of Floridians celebrate their 65th birthday. The population continues to age, chronic disease continues to rise, and families increasingly seek care that allows loved ones to remain safely at home.

While one reimbursement pathway may temporarily be restricted, the need for services certainly is not.

History has shown that the most successful organizations are not those that rely on a single payer or business model — they are the ones willing to adapt as health care evolves.

Florida Offers More Opportunities Than Ever

Many providers who initially planned to pursue Medicare certification are discovering significant opportunities in other sectors of home-based care.

These include:

  • Private Duty Personal Care
  • Homemaker and Companion Services
  • Medicaid Managed Care Programs
  • Long-Term Care Insurance
  • Veterans Care Programs
  • Private Pay Concierge Services
  • Employer-Sponsored Home Care Benefits
  • Hospital-at-Home Partnerships
  • Remote Patient Monitoring and Virtual Care
  • Care Management and Care Coordination Services
  • Dementia and Memory Care Programs
  • Post-Acute Transitional Care
  • Wellness and Preventive Home Services

These service lines not only diversify revenue — they also position agencies to meet consumers where health care is heading.

Diversification is No Longer Optional

The agencies experiencing the greatest success today are intentionally building multiple revenue streams.

Instead of asking, "How do we become a Medicare agency?" many leaders are now asking, "How do we become the home care company our community needs?"

That subtle shift changes everything.

Consumers are seeking comprehensive solutions — not fragmented services.

Families want one trusted organization that can help navigate aging, chronic illness, recovery, caregiver support, transportation coordination, technology, safety, and companionship.

The future belongs to agencies that think beyond individual licenses and focus instead on becoming indispensable community partners.

Innovation Creates Opportunity

Technology is rapidly changing how care is delivered.

Artificial intelligence, remote monitoring, predictive analytics, digital scheduling, caregiver matching platforms, virtual nursing support, and remote therapeutic monitoring are becoming part of everyday operations.

Providers that embrace these innovations today will be better positioned for tomorrow's reimbursement models, regardless of payer.

Innovation is no longer reserved for large health systems.

It is becoming a competitive advantage for agencies of every size.

Advocacy Continues

The Home Care Association of Florida (HCAF) continues to work closely with federal and state partners, national organizations, policymakers, and industry stakeholders to monitor the impact of the enrollment moratorium and advocate for policies that protect access to care while ensuring qualified providers have pathways to serve Florida's growing population.

We understand the concerns many entrepreneurs and expanding agencies are experiencing.

Your voice matters.

Our advocacy continues.

The Florida Opportunity

Sometimes our greatest growth comes when circumstances force us to think differently.

Florida remains one of the nation's strongest home care markets.

The demographics are undeniable.

The workforce continues to evolve to meet changing needs.

Health care continues shifting toward the home.

The organizations that will thrive over the next decade will be those willing to pivot, diversify, innovate, and invest in meeting consumers' changing expectations.

This is not the end of opportunity.

It is the beginning of a new chapter.

Looking Ahead

At HCAF, we believe home care is no longer defined by a single license or reimbursement source.

It is defined by the ability to improve lives where people most want to receive care — their homes.

Whether your organization provides skilled nursing, personal care, companion services, therapy, remote monitoring, care coordination, or innovative aging services, your work matters.

The future of health care is being built in the home.

Florida providers have always led with innovation, compassion, and determination.

Now is another opportunity to lead the way.

Together, we will continue to spark what's next.
