Originally developed for use in early 2000, the Outcome and Assessment Information Set (OASIS) has changed and grown over the last 23 years. No longer is it just a patient assessment tool. Your agency's outcomes and reimbursement rely heavily on how well this information is documented from the beginning to the end of each episode.

During this OASIS workshop will explore all the details of OASIS-E. By using the current OASIS guidance manual, examples, and real-world patient scenarios, you will learn how to formulate a plan for your agency to improve patient assessment, outcomes, and reimbursement.

Learning Objectives

After this workshop, the active participants will be able to:

• List four uses of OASIS data within the home care agency.
• Verbalize understanding of how OASIS data is collected, used to calculate outcomes, and where this information is reported and accessed by both the home care agency and the consumer.
• Explain the eight specific OASIS items used in the calculation of the Patient-Driven Groupings Model (PDGM) functional level adjustment and how these points are translated.
• Formulate a plan to educate clinical staff on the top three areas of OASIS deficiency within their agency through a review of OASIS items, CASPER (Certification and Survey Provider Enhanced Reports), OASIS scrubbers, and other agency resources.

Meet the Speaker

Apryl Swafford
Quality Assurance (QA) Manager, SimiTree

Apryl Swafford, who currently serves as QA manager for SimiTree, has more than 20+ years of experience in home health. She began her nursing career in 1994 with a BSN from Jacksonville State University in Alabama and has worked in roles as a case manager, clinical manager, and leader of a cardiopulmonary specialty program. Apryl is a certified OASIS specialist (COS-C) as well as having coding certifications in both home health and hospice (HCS-D, HCS-H). She has led multiple coding and OASIS classes for agencies, as well as speaking on topics of interest to home care and hospice agencies, such as OASIS E, home health regulations, PDGM, outcomes management and others, at many state associations.

As QA manager for SimiTree, Apryl monitors the overall quality of coding, OASIS, and plan of care reviews completed by both full-time and contract auditors. Apryl works in conjunction with the QA specialists and education department to identify topics of education to address any trending issues noted in QA reviews. Apryl also assists with onboarding new auditors as needed and assists team managers with completing coding and OASIS reviews when volumes increase.

Apryl has provided several articles for home care publications, written scenarios for auditor training as well as OASIS classes, and been a subject matter expert for home care articles in the past few years.

Continuing Education

Attendees receive 14.0 continuing education units if they attend the full workshop for skilled nursing, occupational therapy, physical therapy, and speech therapy. This program is approved for 11 HCS-O BMSC CEUs.
