Since 2013, HCAF has been honoring exceptional home care professionals with the Excellence in Home Care Awards, recognizing those who provide outstanding service and quality care. These awards highlight individuals who epitomize the essence of home care, putting others' needs first.

We invite you to nominate caregivers, leaders, clinicians, or advocates who consistently demonstrate excellence and positively impact the lives of others.

The nomination process is quick and easy, allowing you to celebrate the positive aspects of the home care industry and honor those driven by genuine compassion. A committee of industry leaders carefully reviews nominations, and winners will be announced and celebrated at a special ceremony during HCAF's 35th Annual HomeCareCon Annual Conference & Trade Show this summer.

Nominations for the listed award categories close on Monday, April 26, 2024.

• Terri Santangelo Outstanding Leader Award – Named in honor of HCAF's beloved former president, Terri Santangelo, this award recognizes a clinical or agency leader who embodies Terri's leadership qualities and inspires staff to achieve excellence in home care service.
• Outstanding Professional Caregiver Award – This award celebrates an exceptional clinician actively engaged in daily home care services, consistently delivering outstanding care to patients in their homes.
• Outstanding Paraprofessional Caregiver Award – This award celebrates the invaluable contributions of paraprofessional caregivers who play a vital role in home care services.
