The Centers for Medicare & Medicaid Services (CMS) has released the December 2025 Home Health Value-Based Purchasing (HHVBP) Model Newsletter, which includes performance reporting updates, final payment-year results, insights from the Calendar Year (CY) 2026 Home Health Prospective Payment System (PPS) Final Rule, and new educational resources for home health agencies participating in the expanded HHVBP Model.

What’s Inside

  • Interim Performance Reports (IPRs) – October 2025 preliminary reports now in Internet Quality Improvement and Evaluation System (iQIES); includes CY 2025 measure set results and a preview of CY 2026 benchmarks and thresholds.
  • Annual Performance Reports (APRs) – Final CY 2025 APRs are posted and show each agency’s Adjusted Payment Percentage (APP) for CY 2026 claims, with detailed measure scores and point calculations.
  • CMS Call for Information – CMS is seeking examples of how HHVBP performance results are being used by referral partners, state Medicaid agencies, and managed care organizations.
  • CY 2026 Home Health PPS Final Rule – Highlights finalized changes to the CY 2026 HHVBP measure set, including removal of three Home Health Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey measures and the addition of four new measures.
