CMS Releases Updated Home Health Change of Care Notice (HHCCN) Form Effective February 1, 2025
On December 3, 2024, the Centers for Medicare & Medicaid Services (CMS) announced updates to the Home Health Change of Care Notice (HHCCN) form (CMS-10280). Effective February 1, 2025, Medicare-certified home health agencies must use the revised HHCCN to notify Medicare fee-for-service (FFS) patients of changes to their plan of care.
Changes to the HHCCN Form
- Improved Readability and Design:
- The form has been redesigned to enhance clarity and ease of use.
- Updates follow research-based, plain-language standards to improve communication with patients.
- Expanded Language Options: The HHCCN is now available in Chinese, Vietnamese, and Korean, in addition to English and Spanish, increasing accessibility for diverse patient populations.
- Implementation Timeline:
- Providers may continue using the current HHCCN through January 31, 2025.
- All providers must implement the revised HHCCN starting February 1, 2025.
