New HHVBP Interim Performance Reports Available in iQIES for October 2025

The Centers for Medicare & Medicaid Services (CMS) has released the Preliminary October 2025 Interim Performance Reports (IPRs) for the expanded Home Health Value-Based Purchasing (HHVBP) Model in the Internet Quality Improvement and Evaluation System (iQIES). These quarterly reports provide each home health agency’s cohort assignment, 12 months of performance data, and interim Total Performance Score (TPS), allowing agencies to monitor performance relative to peers.

The Calendar Year (CY) 2025 measure set is now fully in effect, including:

  • Outcome and Assessment Information Set (OASIS)-based measures
  • Claims-based measures (Discharge to Community – Post-Acute Care and Potentially Preventable Hospitalizations – Post-Acute Care)
  • Home Health Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey-based measures

Note: HHCAHPS® measures do not count toward TPS for smaller-volume agencies.

Home health agencies receive an October 2025 IPR if they were Medicare-certified before January 1, 2024, and meet the minimum data thresholds for at least one measure category.

Important: CMS corrected the CY 2024 improvement thresholds for the OASIS-based Discharge Function measure in the July 2025 IPRs. Home health agencies should review the “Baseline Year Data Period” field in their July IPR:

  • If it shows 12-31-2024, the threshold was corrected, and the October IPR contains the accurate value.
  • If it shows 12-31-2023, no correction was needed.

Starting with the October 2026 IPRs, CMS will begin scoring agencies using the new CY 2026 HHVBP measure set. To support readiness, the October 2025 IPRs now include achievement thresholds, benchmarks, and improvement thresholds for all CY 2026 measures. 

IPRs are available in iQIES under the Home Health Agency Provider Preview Reports folder. Instructions on how to access the IPRs are also available on the Expanded HHVBP Model webpage under “Model Reports.” Home heath agencies have 15 days (until December 16, 2025) to submit a recalculation request if they believe the Preliminary IPR contains calculation errors.
