Public Reporting Update: October 2025 Home Health Quality Reporting Program Refresh Now Live

Following the federal government shutdown, the Centers for Medicare & Medicaid Services (CMS) has resumed routine data updates. As of November 20, 2025, the October 2025 refresh of the Home Health Quality Reporting Program (HH QRP) is now live on both the Medicare.gov Care Compare tool and the Provider Data Catalog (PDC).

What This Refresh Includes

The updated data reflect quality assessment submissions from home health agencies (HHAs) across multiple reporting periods:

  • Outcome and Assessment Information Set (OASIS)- and Assessment-Based Measures: Quarter 1 (Q1) 2024 through Quarter 4 (Q4) 2024
  • Discharge to Community and Medicare Spending Per Beneficiary: Q1 2023 – Q4 2024
  • Potentially Preventable 30-Day Post-Discharge Readmission: Q1 2022 – Q4 2024
  • Home Health Within-Stay Potentially Preventable Hospitalization: Q1 2024 – Q4 2024
  • Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS): Q2 2024 through Q1 2025

These datasets help consumers, payers, policymakers, and home health providers evaluate quality and performance trends across the industry.

Temporary Measure Suppression

Due to technical issues, CMS has suppressed two measures for this update:

  • Transfer of Health Information to the Provider
  • Discharge Function Score

Both measures are expected to return with the January 2026 refresh once data accuracy is fully validated.

Home health agencies and stakeholders can view the refreshed October 2025 HH QRP data at Medicare.gov Care Compare or the Provider Data Catalog.
