Strengthening Community-Based Palliative Care Through Quality Standards and Practical Tools

This session offers a foundational understanding of developing a high-quality, community-based palliative care program for adults and children. Community based palliative care case examples will highlight distinct aspects of community-based care and the essential role of the inter-professional teams. The discussion will explore how the National Consensus Project (NCP) Clinical Practice Guidelines for Quality Palliative Care —which ground accreditation standards and ACHC resources—can serve as a framework for quality improvement and palliative care program development. Resources from CAPC, the Alliance for Care at Home, and ACHC will be shared.
At the end of this session, participants will be able to:
- State three essential components of high-quality, community-based palliative care.
- Discuss the use of NCP Clinical Practice Guidelines to creating a quality program.
- Identify CAPC/Alliance/ACHC resources to support the design and implementation of a sustainable palliative care program.
