The Centers for Medicare & Medicaid Services (CMS) Home Health Value-Based Purchasing (HHVBP) Model Technical Assistance team invites Medicare home health providers to participate in a brief 15-minute survey designed to learn more about providers' experience with the resources provided for the expanded HHVBP Model and identify areas for future outreach and education.
