On July 16, 2024, the Agency for Health Care Administration (AHCA) announced that updated versions of the Florida Medicaid Provider Agreements will take effect starting July 15, 2024. These new 2023 forms will replace the existing 2013 versions currently in use and will be accessible on the Medicaid Web Portal under the Enrollment Forms section.

There will be a two-month grace period from July 15, 2024, to September 14, 2024, during which both the old 2013 forms and the new 2023 forms will be accepted for new applications and renewals. After September 14, 2024, only the 2023 versions of the forms will be accepted.

Updated Medicaid Provider Agreements

• Non-Institutional Medicaid Provider Agreement
• Institutional Medicaid Provider Agreement
• Medicare Crossover-Only Provider Agreement
• Institutional Medicaid Provider Agreement – Intermediate Care Facility for the Developmentally Disabled (ICF/DD)
• Institutional Medicaid Provider Agreement for Qualified Hospitals Electing to Make Presumptive Eligibility Determinations

Providers are encouraged to review the updated versions of these agreements as soon as possible. Familiarizing yourself with the new forms will ensure a smooth transition and help avoid any disruptions in your enrollment or renewal process.
